TRANSITIONAL CARE/SHORT-TERM REHAB

Returning home immediately following hospitalization is not always the safest option. Our multi-disciplinary team creates and focuses on an individualized care plan to meet you or your loved one’s specific needs and goals. Our therapy team guides treatment and progress to ensure rehabilitation milestones to recovery are being met. We provide care to treat complex diagnoses that result from illness, disease, surgery, disability or  injury. We encourage resident and family participation while setting therapy goals to aim on helping you or your loved one return home, as quickly and independent as possible by utilizing one or a combination of therapies that may include:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy

Each individualized care plan for rehabilitation will strive to achieve you or your loved one’s goals for increased strength and mobility, enriched everyday living skills, and improved communication and/or swallowing. Common illnesses or overall health challenges that may require services in a short-term rehabilitation facility may include:

  • Pneumonia
  • Urinary Tract Infection (UTI)
  • Orthopedic Procedures or Joint Replacement
  • Exacerbated Heart Failure, Kidney Failure or Complications with COPD Causing Severe Weakness
  • Sepsis
  • Stroke (CVA)
  • Heart Attack
  • Fractures
  • Surgical Procedure Requiring “In-Patient” Aftercare
  • Severe Infections Requiring IV-Antibiotics
  • Respiratory Failure
  • Repeated Falls with Injury
  • Severe Wounds
  • Post Cardiac Event or Surgery
